These sample disclosures can be included as part of an agreement related to credit entries subject to UCC Article 4A. There are two examples of sample agreements in this document: 

• A Notice by the ODFI to the Originator for Non-Consumer Credit Entries and
• A Notice from the RDFI to the Receiver for Credit Entries Subject to Article 4A
